Requirements for LPN Transition to Practice Program

  • Graduation from an accredited nursing program
  • LPN Certificate
  • Current Massachusetts Nursing License
  • Less than one year of ambulatory care nursing experience

Program Summary:This position is a time limited educational experience to introduce the new graduate LPN to nursing practice from multiple perspectives through didactic, clinical and experiential opportunities. Participates in all didactic sessions as part of the role, as well as learning to participate in the planning, delivery and management of direct patient care utilizing the nursing process and adhering to the standards of nursing practice.

Program Description:The LPN Transition to Practice program provides recent graduates of nursing schools with an evidence-based curriculum of didactic and clinical skills to support the transition to Ambulatory practice. During the program, participants will attend classes, complete patient care with an experienced preceptor, and participate in simulation to further develop critical thinking skills and increase confidence in safe delivery of care.

LPN Job Description:

I. Major Responsibilities:
1. Assists in patient assessment according to standard procedure and needs of patient and their families.
2. Administers medications, performs treatment therapies, and operates equipment according to policy and procedure.
3. Assists in implementation and evaluation of care plan including long and short-term goals, nursing actions and teaching.
4. Documents patient care according to standard procedure. Records and reports observed. symptoms, reactions, treatments and changes in patient condition to L.I.P. and or R.N. and assists in analyzing and interpreting patient data.
5. Assists physician with treatments, procedures, and special tests as required or directed. Collaborate with other health care providers to ensure quality and continuity of care.

II. Position Qualifications:

License/Certification/Education:
Required:
1. Completion of an accredited school of practical nursing program.
2. Current Massachusetts licensure required.

Experience/Skills:
Required:
1. Knowledge of basic nursing procedures.
2. Strong interpersonal skills are required to gather information from individuals, build relationships that support collaborative work within the department and across the organization and provide exceptional service to patients.
3. Ability to take direction from a patient or provider, adjusts equipment, observe and describe patient observations.
