Community Healthlink (CHL) is looking forPhysical Therapists- Early Intervention who are interested in working (12-24 hrs/wk). Flexible hours available - days, early evenings and/or Saturdays on a fee-for-service basis in the North Worcester County area.

TheEarly Intervention (EI)team provides strength-based, family-centered services for young children under three years of age who have developmental delays or are at risk of a developmental delay. Services are designed to meet the needs of each infant or toddler, and the needs of their family. The EI team provides services in the home/community setting with the goal of enhancing the child's physical, cognitive, communication, social-emotional and adaptive skills.

Responsibilities:

  • Provide direct intervention services to children 0 - 3 and their families.

  • Carry out service coordination functions when appropriate as defined by the Early Intervention standards.

  • Assist in transition planning prior to discharge from the program.

  • Referrals to outside agencies as needed.

  • Maintain client records and other administrative duties according to DPH and Lipton Early Intervention policies.

  • Attend staff and/or region meetings as required.

  • Attend in-service education and professional meetings as required.

  • Perform related duties as assigned.

  • Adhere to all of CHL's protocols, policies, and procedures.

  • Early Intervention services are provided both in-person and/or by Telehealth.

  • Maintain regular, reliable, and predictable attendance.

  • Maintain confidentiality.

  • Ability to work independently and contribute to the overall success of the team.

  • Flexibility, excellent interpersonal skills, verbal, written and communication skills.

  • Respect diverse views and approaches, demonstrate Standards of Respect, and contribute to creating and maintaining an environment of professionalism, tolerance, civility and acceptance toward all employees, patients and visitors.

Qualifications:

  • Bachelor's Degree or higher required

  • MA PT license required

  • CPR/First Aid certification or obtain within 6 months of employment.

  • Department of Public Health CEIS certification or eligible for certification.

  • NPI or apply for NPI (National Provider Identifier Number).

  • Minimum of 2 years experience working with young children and families preferred.

  • Evaluate and treat children with musculoskeletal, neurological and/or cardiopulmonary deficits which may block normal movement.

  • Demonstrate knowledge of typical infant/toddler development in perceptual/fine motor, gross motor, cognition, language and communication, self-care and personal/social areas.

  • Demonstrate the ability to support positive child and family development.

  • Develop functional intervention techniques appropriate for infants and toddlers with atypical development.

  • Current valid US-issued driver's license and ability to provide a registered and insured automobile for work related purposes, including travel between work sites.

  • Must be able to pass a CORI background check.
